be included with every abstract.  An acknowledgment of each abstract and its
payment will be sent upon receipt. Abstracts received without payments will be
returned.  There are no abstract refunds.

Please submit original abstract plus one photocopy with this completed form
according to instructions in the Preliminary Announcement of the meeting. 
There is no exception to submission deadline.
